July 1 Immunization Record Deadline for New Students

The Registrar’s office sent out the following email today to all incoming students.  If you are the parent or guardian of a new student, please be aware of this July 1 deadline.

Members of the Incoming Class,

This is friendly reminder that your required immunization records MUST be on file at Wake Forest University with the Student Health Service Office by July 1.

Please go to http://newstudents.wfu.edu/general/requiredimmunizatons/ to read more about the immunization documentation requirement.  You can obtain the required 2012-2013 Health Information & Immunization Form at this site.  If you have questions about immunization records please contact the Student Health Service Office by calling (336-758-5218) or e-mailing at (shs@wfu.edu).

If you have completed this requirement or you are currently working with the Student Health Service Office to fulfill the requirement, you are fine.   But if you have not supplied the Student Health Service Office with your required immunization records…the deadline is July 1.
