The following message was sent to any new undergraduate, graduate, and professional students who are not yet compliant with all immunization requirements

—————————————-

This message is being sent on behalf of University offices involved with immunization compliance.

Bottom Line, Up Front:

  • This is an urgent immunization compliance notice.
  • Your immediate action is required.
  • If you need a COVID-19 vaccine, you must secure one off campus.
  • For all other vaccines besides COVID-19, call the Student Health Service at 336-758-5218 to make your vaccine appointment as soon as possible.
  • You will not be able to remain enrolled at Wake Forest if you do not get the required vaccines as soon as possible

University records indicate that you have not complied with all of the University’s immunization requirements.

Students who remain non-compliant will have their Fall 2022 class schedule dropped and they will be removed from university housing (if applicable).

As soon as possible, check the Student Health Portal to see which vaccines you need:

  • Log into the Student Health Portal with your WFU email address.
  • Look at the IMMUNIZATION COMPLIANCE banner that appears at the top of your screen (pictured below; click to enlarge).
Example of the new student portal banner
  • Red Xs for required vaccines in the IMMUNIZATION COMPLIANCE banner indicate which vaccines you need to receive.
  • If you need a COVID-19 vaccine or booster:
  • If you need any other vaccine that is not for COVID–19:
    • Call the Student Health Service at 336-758-5218 and ask to schedule a vaccine appointment so you can come into compliance with University requirements.
    • Note your appointment day and time. You must show up for your appointment as scheduled.
    • The Student Health Service will update your record; you will not have to upload vaccine documentation for these vaccines.
  • If you had already gotten your vaccine(s) and had previously tried to upload your documentation to the Student Health Portal, check your Secure Messages in the Student Health Portal to see if any information was missing from your submission and provide an updated version.
    • Please allow 2-3 business days from when you submitted your documentation before checking to allow for processing. Additional details can be found here.
    • If there are no secure messages, your original documentation was not received and you will need to upload your vaccine documentation again.
    • If you lost your vaccine documentation, call the Student Health Service at 336-758-5218 and ask to speak to someone about what to do for lost vaccine documentation.
  • In the event you see all green check marks for required vaccines in the IMMUNIZATION COMPLIANCE banner, your documentation has been accepted and you are in compliance.

It is critical that you come into immunization compliance immediately to avoid cancellation of your classes or housing (if applicable). Please take action now.
