The following email was sent out to upperclass students regarding fall health insurance enrollment.  Parents and families, please be sure to take whatever appropriate action your student needs regarding his/her health insurance.

Dear Wake Forest Students:

Health insurance is required as a condition of enrollment for full-time, degree seeking students. Visit the student insurance web page for complete eligibility requirements and plan details.

If you desire enrollment in Student Blue, please actively enroll yourself. By completing this enrollment, you will be able to request that the insurance card be mailed to the address you prefer as well as ensure that you receive your card in a timelier manner than with automatic enrollment.

If you wish to waive coverage you MUST complete a waiver by Monday, Aug. 29, 2011. Failure to complete the waiver by Aug. 29, 2011 will result in automatic enrollment into Student Blue. INSURANCE WAIVERS WILL NOT BE ACCEPTED AFTER AUG. 29, 2011.

TO ENROLL:

1)     Visit the student insurance web page

2)     Select “Enrollment/Waivers” link on the left side of the web page

3)     Read the enrollment, verification and waivers information

4)     Follow the blue link to the Blue Cross Blue Shield NC web site

5)     Select “I need insurance”

6)     Click on “Enroll now”

7)     Continue to select appropriate links and complete information as requested

TO WAIVE:

1)     Visit the student insurance web page

2)     Select “Enrollment/Waivers” link on the left side of the web page

3)     Read the enrollment, verification and waivers information

4)     Follow one of the two links in blue to the Blue Cross Blue Shield NC web site

5)     Select “I have insurance”

6)     Click on “Waive Coverage”

7)     Continue to select appropriate links and complete information as requested

8)     Upon completion, you will see a confirmation page and receive an email. If you don’t receive a confirmation, you did not complete the form.

To waive coverage, your current health insurance must meet the following criteria:

1)     Have a lifetime maximum benefit of at least $1,000,000. Plans with per incident, per illness or maximums that are less than the lifetime maximum are not acceptable.

2)     Provide access to health care providers in the Winston-Salem, NC area for emergency and non-emergency conditions, including mental health care.

If you have an HMO and Winston-Salem, NC is out-of-network, your coverage doesn’t meet our criteria. If your HMO offers guest privileges, you must contact your insurer to receive guest privileges and do so prior to waiving coverage.

3)     Provide prescription drug benefits.

4)     Will remain in effect for the 2010-2011 academic year.

International students with F or J visas will not have the option to waive from the Student Blue plan unless they are covered by a plan reviewed and approved by the University based on the minimum criteria.

Your initial insurance decision will apply to the entire 2011-2012 Academic Year (Fall, Spring and Summer).
